Its been a while since I've done it, but there is a short ferry crossing from northern Germany to Denmark from memory. Once in Denmark you can drive across to Sweden & then on to Norway. I thought the roads were ok. You will need winter tyres & possibly snow chains, shovel etc by law. I'm not sure about tolls. I'm sure someone will be more up to speed on tolls & laws etc.

You can take the E20 instead of ferry.

You can go 2 ways, either as mentioned the ferry from Puttgarden Germany to Rodby Denmark. If you go north east from Hamburg you will see Puttgarden, the ferry costs around 75 euro for a lwb van, they run about 2 every hour and only takes about 30 mins crossing, you can get a nice meal on there. When you come off drive up towards Copenhagen and you then take the long bridge across the water to Sweden, cant remember how much it is but its not overly expensive. If you really dont want to do the ferry then you can go from Hamburg up to Kolding and then east to Copenhagen. There is a toll bridge across that way, cant remember how much but again not too expensive. You have no road tolls in Denmark or Sweden apart from the mentioned bridges. There are some road tolls in Norway but not that many it depends where you are travelling. If you go into Oslo they have automatic cameras that record your number and then you will receive a bill at your uk address for the charges from going into Oslo. This was how they used to do it but they may have changed it so best to check. There is a good website www. Highwaymaps. eu where you just select the country and it will give you all the info you need. As for driving out there, its no worries the roads are fine. Remember you need your lights on all day in Denmark and Sweden, Good luck if you go.
